FDA Approves Conditional Silicone Breast Implants

On Wednesday 4/14/05, the FDA health advisors recommended removing the 13 year old ban on silione-gel breast implants. This move came about as Mentor Corp. persuaded the FDA that its newer silicone implants are much more safe and durable than older versions. The company may use these implants only under the following safety conditions set forth by the FDA.

- Prospective patients must sign a consent form acknowledging that they understand the risks of a silicone breast implant including the fact that they may break and require replacement or removal.

- Mentor may only sell silicone breast implants to board-certified plastic surgeons who complete a special hands-on training program to learn how to insert the implants in a way that minimizes the odds of it breaking.

- Mentor must create and maintain a registry to track the long term results patients have with the implants.

- Patients must be educated about the fact that if an implant breaks, the effect of the breakage generally does not cause immediate symptoms. Further they are advised to have an MRI scan after five years and every two years thereafter to check for breakage.

- Mentor must conduct a 10 year study to determine the percentage of breast implants that will break in that time.

- The study must be reviewed by an independent data monitor.

- The results of Mentor's breast implants will be reviewed by the FDA in five years to verify that the implants are performing as expected.

Silicone-gel breast implants began selling in 1962 and were banned in 1992 amid health fears. Research since then has shown that they do not cause serious diseases like cancer or lupus but that they can cause infection and scar tissue if they leak and are untreated.

Approximately 264,000 breast enlargements and 63,000 breast reconstructions were performed in the United States in 2003 - the vast majority using salt water filled implants that are sold without restriction. It is estimated that if the silicone breast implants return to the market, 200,000 women may have them installed within the first year.

The question for both genders is: 'how much progesterone is your body producing to help keep you hormonally balanced.'

The key word here is "balance." As we get older we tend to produce less and less progesterone for a variety of reasons, such as menopause, creating a hormone imbalance that can affect us in many ways.

There is a big difference between pharmacological versus physiological doses of a hormone.

A medical abortion is a type of non-surgical abortion in which a drug is used to induce the abortion.

Procedure, Medical

Initial appointment, which includes pregnancy test by blood and/or ultrasound, informed consent form, and STD tests if necessary.

  • An injection of methotrexate or a dose of mifepristone in tablet form.
  • A prostaglandin analog--a dose of misoprostol in tablet form, or gemeprost taken within 72 hours of the previous medication.
  • A follow-up appointment to make sure the abortion was complete.

Mechanism Of Action.

Each medical abortion drug has a unique mechanism of action.

  • Mifepristone binds to the progesterone receptor to block progesterone. Progesterone is necessary for pregnancy maintenance. Mifepristone also softens and dilates the cervix, and causes decidual necrosis (which leads to placental detachment). In addition, it increases prostaglandin release from the uterine lining, increases uterine contractions, and enhances uterine sensitivity to prostaglandin.
  • Methotrexate blocks an enzyme necessary for DNA synthesis, thus inhibiting the growth of rapidly dividing placental trophoblastic cells.

Both mifespristone and methotrexate are followed up with a prostaglandin drug in medically approved medical abortion regimens--misoprostol or gemeprost.

  • Misoprostol works by softening and dilating the cervix, and binding to myometrial cells to cause strong uterine contractions. Contractions cause expulsion of the embryo.

Misoprostol has been used alone for abortion since the 1970s. Alone, it has a higher rate of incomplete abortion than when combined with mifepristone or methotrexate. There have been reports of infections, such as toxic shock syndrome, following medical abortions,[citation needed] but the risk of such infection is relatively rare.

Availability

n the US, medical abortion is available at women’s health care centers, private clinics, and Planned Parenthood . The price ranges from about $350 to $610, depending on which medical tests are required.

In Canada, availability is much the same, however in some provinces the procedure is fully covered by the provincial health care plan. Prices in areas where abortion is not covered by provincial health care usually range from $250 to $400.

History

Mifepristone was first licensed in France in 1988. It is now approved in 29 countries, including Austria, Belgium, China, Denmark, Finland, Great Britain, Israel, Norway, Russia, South Africa, Sweden, and Taiwan. In 1993 President Clinton signed an executive order to encourage testing, licensing, and manufacturing of mifepristone in the United States. In 2000 the US Food and Drug Administration (FDA) approved the use of mifepristone for medical abortion in the United States.

It is currently possible to get mifepristone (the most common drug used in combination with a synthetic prostaglandin for medical abortion[citation needed]) in 60 clinics/centers in the United States.

Clinical trials of RU486 are currently being done in Canada in the cities of Edmonton, Winnipeg, Ottawa, Toronto, Montreal, Halifax and St. John's Newfoundland. The drug cannot be imported for personal use. However, medical abortions using Methotrexate and Misoprostol are available in Canada.

Late-stage Breast Cancer Have Lowest Vitamin D

Women with late-stage breast cancer have lowest levels of vitamin D, study finds

(NaturalNews) According to new research published in the Journal of Clinical Pathology, women with advanced-stage breast cancer have significantly lower levels of vitamin D in their bodies than women in the early stages of the disease.

A team of scientists from Imperial College London conducted a study of 279 women with breast cancer. The disease was in the advanced stage in 75 of the women, and in its early stages in the remaining 204 women.

The researchers found that the women with early-stage breast cancer had considerably higher circulating levels of vitamin D in their bodies compared to women in the advanced stages of the disease. The exact reasons for the difference were unclear, and the researchers did not know if the lower vitamin D levels in advanced-stage women were a consequence of the cancer or a cause.

Previous research has indicated that low levels of vitamin D in women may cause breast cancer to progress to more advanced stages, and studies have shown that having adequate levels of circulating vitamin D can reduce the risk of developing the disease. Laboratory research has found that vitamin D -- which can be obtained through sunlight exposure or consuming foods rich in the vitamin, such as oily fish and eggs -- enhances cancer cell death and prevents cancer cells from dividing.

"This report, while being an observational study, clearly shows that circulating vitamin D levels are lower in advanced breast cancer as compared to early breast cancer," said Dr. Carlo Palmieri of Imperial College London. "It lends support to the idea that vitamin D has a role in the progression of breast cancer.

Palmieri said more research was needed to determine the exact causes and underlying mechanisms behind the differences in vitamin D levels in early- and late-stage breast cancer patients.

"We also need to look at the potential clinical implications of monitoring and maintaining high circulating vitamin D levels in breast cancer patients," Palmieri said. "By answering these questions, we may be able to improve the treatment of women with breast cancer."

How your body goes through during Menopause

We need to take a quick look at what Perimenopause and Menopause is and then by doing so we will be better equipped at handling what may come our way. Or if already there, you may have a better understanding of what is going on with your body. You may be forgetful, emotional, tired and experiencing strange heart palpitations. These can all be due to Menopause. Let’s look at some more symptoms.

To put it simply during Perimenopause the hormones in a women’s body changes. Menopause means the end of your menstrual cycle. For some that may be exciting while for others a start of a nightmare. Periods cease because there are such low levels of estrogen and progesterone being produced and eventually they stop all together. With the cease of periods you can also no longer have anymore babies.

The average age for menopause to start is 51 but women can experience menopause either before or after or in an incidence when their ovaries need to be removed. Perimenopause is what your body goes through as it sets in for “the change”. It marks the end of your baby producing years. For every woman how long it lasts is different. It may be a couple of months or a good few years.

A first sign of perimenopause is when your menstrual cycle starts to change. They may become more frequent, less frequent, you may experience a heavier cycle or a lighter cycle, and you will just generally notice a change.

The most common symptom that you will experience is that of Hot Flushes. These flushes will come suddenly and in waves and may be accompanied with feelings of nausea, dizziness and headaches. The hot flushes don’t always last very long and they leave you feeling clammy and uncomfortable. In some women it is just mildly irritating while in others it causes havoc with their day and sleeping at night becomes a constant feat as they are never comfortable enough due to having to deal with night sweats. Night sweats have been known to leave the individual and the bed sheets soaked in perspiration.

Some women experience terrible mood swings and depression. This may be caused due to sleep disruptions that come with having to endure night sweats. It may also be due to a change in the hormonal balance in a women’s body. Either way if it is something that you not feel you have control over you must seek medical assistance. The problem is that because mood swings and depression are so rife it is not often attributed to being just one of those things that come with periimenopause.

Depression can be very debilitating so the quicker you are able to get to the root of it and have access to the love and support you need then the better it is for you and those around you that love you very much. It needs to be remembered that a women has a lot of other factors to deal with at this time of perimenopause, her children leaving home and her having to deal with an “empty nest” is just one of them.

You may need to urinate more often and you may feel pain when you do. This is due to the lining of the bladder becoming thinner. Low estrogen levels in your body may also leave you to be more susceptible to urinary tract infections and the muscles that control the bladder may become weaker. Leakages may occur when coughing or sneezing but there are ways to prevent this from happening when you consult your doctor.

Bones become weaker and osteoporosis may set in. The statistic is that more women are likely to develop osteoporosis than men are because their skeletal frame is generally just smaller. As your bones become weaker, and because of the loss of calcium, due to your body absorbing less, they become brittle and are more susceptible to breaking. Estrogen seems to play an important part in the health of ones bones, as estrogen levels decrease so the bone mass decreases. Hip fractures occur more in older women and it is very difficult and sometimes not very probable that they will recover from it.

Due to a decrease in estrogen the walls of the vagina become thinner, loosing its elasticity and then becomes painful if lining becomes inflamed, rough and dry. This may also cause sexual intercourse to be painful and /or uncomfortable. The thinning of the vaginal walls also makes your body more susceptible to vaginal infections.

Weight gain during menopause is also common and the area where the most weight is put on is around the waist. Some have stated that studies have shown that a lack of estrogen in the body leads to weight gain while others state that it is more to do with the change in lifestyle. Yet that does not explain why there is a redistribution of fat from the thighs to the stomach. Maybe weight gain has got to do with a bit of both. A proper diet and lots of exercise will help. Remember however that ones metabolism does get slower as you get older.

Being a women menopause and so perimenopause is just one of those things in life that we all have to deal with at one time or another. It is how we deal with it that makes all the difference.

Benefits of a Salt Water Colon Cleanse

Using a salt water colon cleanse, also called a salt water flush, is one of the easiest and quickest ways to clean you colon at home. Not only is it colon cleansing, but it also cleanses your stomach and small intestine as well. In other words it flushes out your entire alimentary canal (digestive system). This master system cleanse removes all of the toxins, accumulated fecal matter, regular feces, parasites, and bacteria from your whole digestive tract.

There are many good laxatives, mixes, enemas, and other colon cleansing products on the market, but the simple salt water flush works well using basic ingredients instead of chemicals or medicines. A salt water colon cleanse is a healthier alternative.

To do a salt water colon cleanse, you will need to prepare yourself by scheduling at least a half or full day to complete the colon cleansing. You also need to skip at least one meal or fast overnight before you do a salt water master cleanse. Because of these reasons, most people prefer to do this system flush on the weekend first thing in the morning on a day they have nothing to do and don't need to leave the house.

The salt water colon cleanse starts with one quart of lukewarm water. Add two or more tablespoons of UN-iodized salt or sea salt and stir or shake it until the salt fully dissolves. (Using table salt will not have the same effect, so don't substitute it for the un-iodized or sea salt.) This formulation works like an oral enema.

Drink this salt water mixture quickly and wait for a few minutes before laying down and massaging your stomach down to your colon area. This helps break up accumulated waste as does crouching over your knees. I suggest a few basic yoga stretches that focus on stretching the front and back of your midsection. Your salt water colon cleansing procedure will be more effective the more you massage and stretch your upper, middle, and lower stomach.

Within thirty minutes to two hours after drinking the salt water, you will feel pressure build up in your colon and you will probably have several bowel movements over the next few hours. If you do not experience pressure and heaviness in your bowels, you either need to add more salt to the water or drink more water to completely flush your digestive system. This colon cleanse will actually clean out your system top to bottom.

What about other infections?

Two sexually transmitted diseases, chlamydia and gonorrhea, also can cause vaginal discharge. These are infections of the cervix that are caused by bacteria. Sometimes the only symptom of these infections is an increased vaginal discharge. Both of these infections can be treated with antibiotic shots or pills.

Should my sex partner be treated?

If you have trichomoniasis, chlamydia, or gonorrhea, your sex partner also must be treated. If you have a yeast infection, your sex partner may not need to be treated. Talk with your doctor to find out. You and your partner should avoid sex until after you have been treated.

Tips on preventing vaginitis

* Do not douche.

* Avoid using feminine hygiene sprays, colored or perfumed toilet paper, sanitary pads or tampons that contain a deodorant, and bubble bath.

* After using the toilet, always wipe from front to back. Doing this may help keep bacteria from your rectal area from getting into your vagina.

* Wear cotton underpants during the day. Cotton allows your genital area to "breathe." Do not wear underpants at night.

* Avoid wearing tight pants, pantyhose, swimming suits, biking shorts, or leotards for long periods.

Breast cancer happens when cells in the breast or a tumor, grow out of control and damages nearby tissue. In women, the most common and fatal type of cancer is breast cancer.

Detection
There are often no symptoms in the early stages. Women should be aware of the screening recommendations and follow them. There are varieties of symptoms that may appear as the tumor grows such as:

- The breast changing in size or shape
- Breast skin becomes pitted or ridged
- Thickening or lump in the underarm or breast
- Discharge from nipple or the nipple turns inward
- Skin on the breast becomes red or scales

If you have any of these symptoms, have yourself examined by a medical professional. This does not mean you have cancer but you defiantly want to have this checked.

Before the age of twenty, is very rare to get it and not often diagnosed in women less than twenty-five years old. The chances of contacting climbs steadily after 25 and peaks around menopause age in women. It increases less after menopause but as they age, the risk to older women gradually increases.

Risk Factors
Nobody really knows what causes this cancer. Some of the elements that are thought to increase the risk are:

Gender: There are more cases of women than men
Weight: Overweight women are at higher risk
Age: From 25 to menopause, the chances increases.
Children: If a woman has not had a child, or had a child after 30
Family History: Women that have a family member that have or had it are at risk.

Male Breast Cancer
Yes, it really does happen. It is certainly not as common as in women but approximately one to 1.5% happens to men. Older men most often diagnosed with it and are between sixty and seventy years old. If a man has had previous exposure to radiation, such as for cancer treatment, their risk increases. Approximately 20% of men with a mother, sister or other close female relatives with breast cancer are certainly at higher risk. Some of the symptoms in men includes swelling or a breast lump, retracted nipple or discharge and scaling or redness of the breast skin or nipple.

Statistics
The statistics are frightening. Each and ever year, over 182,000 women and 16,000 men are diagnosed with breast cancer. Over 400 men and 43,300 women will die from this terrible disease. During their lifetime, one woman out of eight has or will get breast cancer. Most people have family or friends that have or had breast cancer. Always give them your support and encouragement.

Breast cancer has earned the disrepute of becoming the second-most killing type of all cancers that are found in humans, having lost the race only to lung cancer. About one in every ten women has chances of developing breast cancer if precautions are not taken early on. Despite this alarming number of its victims, it is pathetic to know that there is an absolute lack of information on the disease. It is not that there is no written material available about the disease, but most women are too complacent regarding the problem. Breast cancer is the abnormal growth of cells within the breast. These cells would aggregate like a lump, which would be called as the breast tumor. The tumor can be felt from the outside of the breast even when it is still harmless. At this harmless stage, it is called as a benign tumor. Benign tumors can be surgically operable. If that is done, the risk of the advancement of the breast cancer is very negligible. But if the tumor is allowed to go on, then it would continue to the malignant stage. During this stage the cancer would proliferate to such an extent within the surrounding tissues that it would be impossible to remove it completely. When the cancerous cells enter the bloodstream, there is no way out. The result is certain death.

Hence it is important to snuff out the breast cancer while it is still benign, and perhaps that is the only way to keep the cancer at bay once it occurs. But most women do not know that there are various ways to reduce the risk of contracting breast cancers. Since breast cancer can strike just about any woman, it is extremely necessary to pay attention to these methods that can help to prevent the disease. The following are the precautions that women must take to prevent the onset of breast cancer:-

(1) If you are overweight, then seriously work at reducing your weight. Science has proved a direct relation between obesity and breast cancer. At the same time, lean women have a significantly lesser chance of contracting the disease.

(2) Do not introduce any supplements in your body that could change the internal hormonal balance of the body. In women, the two main feminine character deciding hormones are progesterone and estrogen. These hormones maintain the proper balance of the female characteristics, including the shape and the function of the breasts. An alteration in this balance could bring on several female health related diseases, among which breast cancer is just one of them.

(3) Smoking is extremely hazardous in women as it can play havoc with the biological rhythm of the body. Excessive smoking could cause sterility in both the male and the female. But in women, smoking has been found (by statistical analysis) that women who smoke stand at a much higher risk of breast cancer than others. The same can be said about women who consume too much of alcohol.

(4) Walking is considered to be effective in keeping breast cancer away. Walking is a simple exercise, but it can keep the woman’s body fit. Also walking stimulates proper blood circulation in all parts of the body without making the person excessively tired. Apart from walking, other simple exercises like running, jumping, swimming, etc. are also effective.

(5) All women over the age of 40 are at a risk of breast cancer. Hence they should keep their lifestyle healthy. The diet should be kept as natural as possible, and artificial spices and preservatives must be avoided. Elderly women should not lead sedentary lifestyles; they must involve themselves in some kind of physical activity or the other, depending on their lifestyles.

(6) There has been a kind of an association established with high fats and breast cancers. Women whose diet consists of a lot of fats are more susceptible to getting breast cancers than others. For this reason, it is necessary to minimize fats in the diet as much as possible. Instead of high-calorie fats, a low fat diet is preferable. This will also help to keep obesity and cardiovascular problems in check.

Breast cancer prevention is more important than the treatment of the cancer itself, because prevention is always better than cure. The above precautions will greatly reduce the risk of having a cancer, but then they are not sure-shot methods. Medical science has not yet been able to come up with a good explanation as to why some women get breast cancers and some don’t. Hence at the slightest sign of any abnormality with your breasts, it is necessary to seek urgent medical attention. Your vigilance might save your life.

Stages of Breast Cancer - The TNM System

If you've had a breast biopsy, lumpectomy, or mastectomy done and gotten back the results from the pathology lab, you will want to understand what the staging part of the report means to you. Staging is related to treatment options and survival rates.

One of the first questions you may ask is, "Has the cancer spread beyond the tumor, or is it all in one place?" Right now, there is not one simple test that gives you the answer to that question. But there is a system of categorizing cancer into stages, based on three factors. The staging system most widely used is the TNM system.

* T is used for Tumor Size. In the image (mammogram or ultrasound) of the tumor, the radiologist can make a measurement of the tumor's size. Sometime this is hard to do, depending on the angle of the tumor in relation to the image film, or if the tumor is deep inside the breast. A tumor has indistinct, or irregular outer edges, sometimes called a spiculated shape. They aren't always round, and they can be longer than they are wide. The most accurate way to get the size of a tumor is to surgically remove all of it, and then measure it. Tumor Size is divided into four classes: T-1 is from 0 - 2 centimeters, T-2 is from 2 - 5 cm, T-3 is greater than 5cm, and T-4 is a tumor of any size that has broken through (ulcerated) the skin, or is attached to the chest wall.

* N is used for Lymph Node status. Since cancer can travel through your body in your lymph system, it is important to have your lymph nodes that are nearest the tumor tested for cancer and micro-metastasis. There are two ways to check the lymph nodes: by touch, and by surgery. If your lymph nodes are checked by touch, your surgeon will palpate (feel) the skin just above the lymph nodes, and rate them. If the surgeon cannot feel any swollen nodes, the rating is N-0, if the surgeon can feel some swelling and thinks the nodes are negative (not cancerous) the rating is N-1a, and if the nodes are swollen and appear positive (cancerous) the rating is N-1b. If the lymph nodes feel like they are quite swollen and bunched together (rather lumpy), they are rated N-2, or if they are near the collarbone, they are rated N-3. The second way to evaluate lymph nodes is with a sentinel node biopsy.

* M is used for metastasis. If a sample of the nodes have been surgically removed and tested, and are clear of cancer, they are rated M-0, but if they have cancer cells or micrometastasis in them, they are rated M-1.

All of the TNM information will be combined twice, once by the surgeon, and again by the pathologist in the lab. Each expert will give an opinion about the cancer, in terms of its TNM stage. Neither of these opinions will be the exact answer to "what stage is this cancer?" You may need to know more about it, such as

* the hormone receptor status (fueled by a hormone)
* the grade of the tumor (speed of growth)
* and if it has spread (metastasized)
* where it has traveled to in your body

All of these things affect your diagnosis, and will be considered in looking at your treatment options.

Four Stages of Breast Cancer
There are four stages of breast cancer,and your diagnosis depends on the tumor's TNM rating. Here's how the stage and the TNM numbers relate.

* Stage 1 means you have a T-1 tumor and clear lymph nodes with no evidence of metastasis, or T1N0M0.

* Stage 2 has at least three subdivisions, which are a combination of T1, T2, or T3 tumor, positive or negative lymph nodes, and no metastasis; for example T1N0M0.

* Stage 3 also has at least three subdivisions, which come from a mix of any size of tumor, positive lymph nodes or nodes located in the collarbone area, and no metastasis; for example T3N1M0.

* Stage 4 is also called metastatic breast cancer, and is rated by having any size of tumor, positive lymph nodes, and obvious metastasis, for example T1N1M1.

The staging and the TNM systems are always being revised and fine-tuned, so ask your doctor to explain these results to you. Ask questions until you have an answer that you can understand. Your doctor can use a picture or a graph to help give you a clear explanation, or perhaps provide some literature for you to read later.

Planning an Appointment with Your Doctor

When you plan a doctor visit, and you know that you will be discussing your diagnosis, lab results, or treatment plans, you may want to ask someone to come along with you, to take notes. If that's not possible, taking a small tape recorder along is a good idea. Sometimes we when get unpleasant news, it is a shock to our minds or our emotions, and we may stop taking in the rest of the conversation. Having a relative or supportive friend along can really help, both in keeping a record of questions and answers, and by giving you an emotional cushion. Be sure that your friend or relative can keep your health information private, if that is what you wish.

Razors are the simplest method of hair removal, yet experts say are still the best. In tests of electric shavers and epilators at Consumer Reports, female testers said that none of the electric models could convert them from their manual razors. In recent years, companies have done a lot to jazz up the basic blade razor, adding built-in lotion dispensers, vibrating heads and ever more blades. But we seem to have hit the point of diminishing returns for razor features: By and large, these added features amount to gimmicky marketing, testers say. Two or three blades are all you need for a comfortable, close shave.

Gillette's Venus Divine razor (*est. $10) was the only female razor included in a list of seven top picks out of hundreds tested by Which? magazine. The Venus Divine has three spring-loaded blades in a large pivoting head, and three lubricating strips which are supposed to make shaving more comfortable. Two words best describe the Venus' performance, according to reviews: closeness and comfort. Users say it gives a great shave and is easy to hold, and that the swivel head easily contours to the body.

The Venus Breeze (*est. $10) is similar, with three blades, but the Breeze has "gel strips" on either side of the blades that are supposed to eliminate the need for shaving cream or soap. Testers at Redbook magazine liked it, saying the Breeze provides a nick-free shave with no irritation to the skin. For some users, however, the large head was just too big. One Venus tester, Austin American-Statesman Staff Writer Helen Anders, chose the Gillette Venus (*est. $10) as her favorite, saying it shaved better than the tricked-out Diva, a razor with a built-in foam dispenser. There's also a battery-operated version of the Venus, the Venus Vibrance (*est. $13) , and although we didn't see this model tested, reviewers aren't keen on battery-powered shavers in general. We've included the Venus Divine in ConsumerSearch Fast Answers. However, note that the Divine isn't much different from the higher-rated, men's-marketed Mach3 Turbo razor, which also has three blades.

Schick has its own popular razor called the Schick Quattro (*est. $10) and Schick Quattro for Women (*est. $10) , which comes in pink and has a slightly different shape. Both have four blades. We didn't see the Schick razors included in as many tests as Gillette, but in Helen Anders' informal tests for the Austin American-Statesman, she didn't notice much of a difference between the three-bladed Mach3 and Venus, the five-bladed Gillette Fusion and the four-bladed Quattro.

With all of the above razors, you'll have to buy replacement blades, which often cost more than the razor itself. Most replacement blades for Gillette and Schick razors cost between $8 and $10 for a four-pack. Disposable shavers didn't generally fare as well in testing. Only one disposable made it to the top picks in the tests at Which? In Redbook's informal tests, the Bic Soleil disposables (*est. $30 for 24 razors) weren't as well liked. Some of the female testers said the two-bladed razor was awkwardly inflexible and required two passes for a good shave. In tests at Which? magazine, only one disposable razor makes the list of top picks, the Gillette Sensor3 (*est. $12 for eight razors) .

Women who don't care about the color of their razor can check out the Gillette Mach3 Turbo (*est. $10) . Which? gives this straightforward razor top billing. Testers say it gives a very close shave without any gimmicks. The Mach3 Turbo does well in user reviews, too, beating out the five-bladed Gillette Fusion (*est. $10) , which the company claims delivers a closer shave. The Mach3 is also the razor of choice of two barbers interviewed by Helen Anders in her article "Sorting through razor innovations is really splitting hairs." The barbers' intuition is echoed in the best razor tests we found.
Women's electric shavers

Shaving dates all the way back to 30,000 BC, when historians say cavemen took rocks and flint blades to their faces to tame their beards. Time and innovation have changed the tools, but experts agree that the hair removal marketplace is full of gimmicky features that don't necessarily improve upon the basic function of giving a good close shave. They roundly conclude that electric razors don't shave as closely as a basic manual razor. But that isn't the only factor important to women, and the newest crop of shavers offers other features that are appreciated in some tests.

Electric razors designed for women were first introduced in the 1940s. They operate with cutters on a floating head that vibrate behind a screen, chopping off hair at the skin level. Because the cutters don't touch skin, electric razors don't nick or cut the skin as easily as blade razors. This is especially important to women on blood-thinning medication or those with bleeding disorders, but it's also nice for women who are used to shaving daily and want a quick, pain-free shave. Women's electric razors can also be used wet or dry, so they work for touch-ups for spots missed while shaving in the shower.

Panasonic shavers came out on top in the best testing of women's shavers, including the Panasonic Close Curves (*est. $40) . The Close Curves uses pivoting blades the company claims are the sharpest available on women's electric shavers. It can be immersed in water for easy cleaning and has a trimmer for detail work. In one test, the Close Curves tied for first place with the pricier Remington Smooth & Silky Ultra (*est. $55) based on its scores for easy clean-up, noise and features. Users on Amazon.com and other retail websites praise the Close Curves shaver for not irritating the skin or causing ingrown hairs. Overall, it scores four out of five stars on Amazon.com. Those who gave it lower scores say it doesn't shave closely, but others counter that notion. One commented she likes the fact that it can be plugged directly into the wall for recharging, saving counter space.

Keep in mind that there's no particular reason women can't consider a men's electric shaver, but most will want to choose a wet/dry shaver so it can be used in the shower. See our report on men's electric shavers for info on those. One potential downside is that female buyers may end up paying for features they don't need -- such as a pop-up beard trimmer -- on men's models.

The Panasonic Pro Curve ES 7038 -- officially a men's wet/dry shaver -- is the top recommended electric shaver at Which? magazine. The consumer group says it gives the closest electric shave and is very comfortable to use. However, none of the testers were women. The same goes for the report on men's electric shavers at Consumer Reports -- none of the testers were women. Consumer Reports does invite women to test four ladies' electric shavers.

Those willing to pay a little more should consider the Remington Smooth & Silky (*est. $55) . This shaver wasn't tested by Which?, but scored well in another test and among user reviews at Amazon.com. In our last report on women's shavers, it was a Top Pick. The version of the Smooth & Silky shaver tested by Consumer Reports, the WDF 5500 Ultra (*est. $55) , gets mixed reviews from owners, but another model in the line, the Smooth & Silky WDF 6000 Ultra (*est. $70) , fares better. It has two gold-plated foils that float and a titanium-plated trimmer. This is an especially good choice for travelers, as it features worldwide voltage. It comes with a two-year warranty, a year longer than many shaver warranties.

A few electric shavers are proving to be unpopular with users, based on online reviews at Drugstore.com and Amazon.com. The Satinelle Ice (*est. $55) has a different shape. It works with ceramic discs and a removable cartridge filled with cooling fluid that is supposed to be placed in the freezer before use. When the liquid changes color, it's ready to go. Most users are lukewarm on the cooling pack, saying they stopped using it because it didn't help much. But, surprisingly, many maintain the Satinelle is still the least painful yet effective epilator they've tried, even without the cooling pack. The Ice has other features users like, too. It has two speed settings -- the slower one designed for sensitive areas -- and comes with more extras than the other Satinelle models including a cleaning brush, exfoliating body puff and a storage pouch. It carries a two-year warranty.

For those with a bigger budget, the Braun Silk-epil (*est. $70) is a strong choice that offers more features, including a light to guide shaving and massaging- and shaving-head attachments. Braun claims it will keep you hair-free for up to a month. The company's claim seems to be borne out by reviews of this product. A tester for Choice raves that it "was by far the best epilator," noting it was the most expensive of five epilators tested. The Silk-epil received the most number of positive reviews of all epilators on Amazon.com – well over 200 to date – and has managed to keep a very high 4.5-star average rating (out of five stars). Some say it's noisy, just about all say it's painful to some degree, but the consensus among users is very good. Overall, this epilator seemed to be the best performer but was more painful than others.

Perhaps one of the best-known names in epilators, Epilady, receives lower marks in tests. Interestingly, many staunch fans of the Braun Silk-epil are former Epilady users, including one who referred to the Epilady as a "torture device" in an online review.

They may be felt during a pelvic exam, but many times myomas that are causing symptoms may be missed if the examiner relies just on the examination. Also, other conditions such as adenomyosis or ovarian cysts may be mistaken for fibroids.

They usually are not associated with cancer, they rarely develop into cancer (in less than 0.1 percent of cases). Having these does not increase your risk for uterine cancer. Many can appear as a single large tumor or as a cluster of small ones.

They are usually present as a central lower abdominal mass rather than totally involving the adnexa. Ultrasound or magnetic resonance imaging often can differentiate uterine leiomyomata from ovarian tumors, but not always.

Fibroids are benign (not cancerous) tumors of the muscular layers of the womb or uterus. They're extremely common and usually occur as several clusters of different sizes. They begin as small seedlings that spread throughout the muscular walls of the uterus. They can be so tiny that you need a microscope to see them.

They also can change the shape of the uterus as they grow. This can make it difficult to become pregnant or can cause problems during a pregnancy. Many have also been linked with a reduction in the success rate of fertility treatments. Fibroids can dramatically increase in size during pregnancy. This is thought to occur because of the increase in estrogen levels during pregnancy.

Martin Luther King was right on when he said, "Our lives begin to end the day we become silent about things that matter." It is the beginning of the end.

A major source of stress is caused by unspoken feelings, not getting what we want, and not asking for what we want. This stress can cause headaches, insomnia, emotional eating, avoidance, depression, and laryngitis just to name a few. I, myself, have experienced all of the tell tale signs at one time or another in my life.

So, back to the question, is your silence causing you stress? A good question to ask yourself is your life working or not working for you? A good place to start looking for answers is to take a good long look at your relationship with yourself and others.

* How many times do you say YES when you want to say NO? For fear that they may not like you. You valued the other person more than you valued yourself. * How many times have you kept quiet because you were afraid to speak up? For fear of what might happen or not sure of yourself. So, you just sucked it up and paid for it on the inside. * How many times have you spoke your feelings, but did not ask for what you wanted? For fear that you didn't know how to ask or felt you weren't worthy enough. So, you came away unfulfilled and sometimes even resentful for not having spoken up and stated what you really wanted. * How many times have you let others dictate the course of your life because you didn't think you knew enough? So, you lived the life they wanted, not yours, simply because you wanted to please them. * How many times have you settled for less when you could have had more? What do you think doing this did to your sense of self worth?

Think about it. When you become silent, don't express your feelings, and don't speak up, you give away your power and your soul. Now, if this only happened one time it may not be a problem. You learn from the experience and you take care of better care of yourself next time. However, keeping silent time after time causes a slow demise of the quality of your life, not to mention your integrity, dignity, joy, passion, and prosperity in your life. How long are you going to keep saying 'Ow!' before you stop!'

So how do you reclaim your power?

The first step is to be accountable. You are where you are because of the choices you have made in your life. Now don't go south on me and start beating yourself up more. It's not bad or wrong, it's just where you were at the time. Now is the time to make better choices for yourself - stand up for yourself.

The second step is to take a look at the areas of your life where you've sabotaged yourself. Determine what it is that you want and start putting in corrections. In other words, start taking different actions than the ones you took before. And every time you take a new empowered action, you begin to reclaim your power little by little.

Being silent does have its place in everyone's life, but staying silent when you really need to be standing up and showing up for yourself serves nobody well, least of all you. If you're really having problems stating what you want, just remember the old saying, 'How do you eat an elephant? One bite at a time.' In short, open your mouth and clearly state what you want - one word at a time.
